This form allows both pupils and teachers to participate in the Prize Draws, to receive Certificates of Achievement and a Thank You Award. To be eligible for the School Trophy all school collating forms should be returned together. Please do not return individual pupils' forms. Remember Teachers, this completed form is your entry to the great prize draw for a weekend away!
When you have collected the money from the pupils, please fill out the MS READaTHON Collating Form.
Retain 10% of the money collected for the purchase of books or reading aids.
Lodge the remaining money using the pre-printed BANK GIRO form.
