Online collating form

Teachers, when your school has finished reading, you can submit the below Online Collating Form. Please fill in one form per teacher, and submit all forms together. Please do not return individual pupils’ forms.

This completed form ensures entry for your pupils and teachers in the great prize draws, and to receive Certificates of Achievement and Thank You Gifts. Please submit by November 26th to ensure entry in the prize draws (December 3rd for schools that entered after the mid-term).

When submitting:
•Retain 10% of the money collected for the purchase of books or reading aids.

•Lodge the remaining money using the pre-printed BANK GIRO form we posted out to the school (contact us if you need more)

•You can submit this form online below, only if the Lodgement Details are included.

•Only one teacher per Collating Form submission please.
